{"id":2603,"date":"2023-06-15T10:17:45","date_gmt":"2023-06-15T15:17:45","guid":{"rendered":"http:\/\/test.pegasuslectures.com\/blog\/?p=2603"},"modified":"2023-08-15T18:10:33","modified_gmt":"2023-08-15T18:10:33","slug":"bringing-vsi-imaging-to-the-amazon-of-peru-chapter-7","status":"publish","type":"post","link":"https:\/\/www.pegasuslectures.com\/blog\/index.php\/2023\/06\/15\/bringing-vsi-imaging-to-the-amazon-of-peru-chapter-7\/","title":{"rendered":"BRINGING VSI IMAGING to the AMAZON  of PERU (Chapter 7)"},"content":{"rendered":"<p style=\"text-align: justify;\">Today was our first day of training in <a href=\"https:\/\/www.citypopulation.de\/en\/peru\/loreto\/maynas\/1601100016__angoteros_monterrico\/\">rural Angoteros<\/a>.\u00a0 We spent the morning getting everything in place, with the biggest logistical challenge being the need for electricity to conduct our presentation.\u00a0 Power only runs between 6 pm and 10 pm.\u00a0 At 10 pm the lights go out no matter what you are doing.\u00a0 When I say the lights go out \u2013 I truly mean it is pitch black.\u00a0 Luckily the little USB charger fan I brought serves as a back up charger for a phone and as a flashlight.<\/p>\n<p style=\"text-align: justify;\">I got out of bed at 5:30am at the insistence of the local roosters.\u00a0 Whoever said roosters start crowing at sunrise has not met these roosters.\u00a0 The first \u201ccock-a-doodle-doo\u201d I heard was around 3am.\u00a0 Thereafter, a constant rooster symphony let us know they were there and wanted the world to know it.\u00a0 After a few hours of work using my computer on battery power, it was now breakfast time.\u00a0 Before we left, we received an extremely pleasant surprise.\u00a0 Dommi, the <a href=\"https:\/\/www.misje.pl\/o-nas-308\/komisja-episkopatu-polski-ds-misji-309\">Polish missionary<\/a> we met yesterday, had sent over a bag containing bath towels for each of us. This was most welcome, as showering the day before had felt awkward, drying off with no towel.\u00a0 Funny what we take for granted.<\/p>\n<p style=\"text-align: justify;\"><a href=\"https:\/\/media.pegasuslectures.com\/blog\/Picture51.jpg\" target=\"blank\" rel=\"noopener\"><img decoding=\"async\" src=\"https:\/\/media.pegasuslectures.com\/blog\/Picture51.jpg\" height=\"100px\" align=\"left\" border=\"0\" hspace=\"10\" \/><\/a>There are no restaurants here <a href=\"https:\/\/www.citypopulation.de\/en\/peru\/loreto\/maynas\/1601100016__angoteros_monterrico\/\">in Angoteros<\/a>, and only a few tiny stores called \u201ctienditas\u201d (translation: \u201clittle shop\u201d).\u00a0 I have included a picture from inside one so you can see what shopping is like and what purchase options exist.\u00a0 The people in this village of less than 800 people live primarily off what they grow and what they can hunt or fish.<\/p>\n<p style=\"text-align: justify;\">As we walked over for breakfast, a small movement in a tree caught my eye.\u00a0 See if you can find what caused it in this next picture.<\/p>\n<p style=\"text-align: justify;\">In case you did not spot it \u2013 here is a zoomed picture.\u00a0 A little girl was standing up in the tree. I am not sure if she was up there to get a glimpse of the strangers in town, or simply as a fun activity.<\/p>\n<p style=\"text-align: center;\"><a href=\"https:\/\/media.pegasuslectures.com\/blog\/Picture52.jpg\" target=\"blank\" rel=\"noopener\"><img decoding=\"async\" src=\"https:\/\/media.pegasuslectures.com\/blog\/Picture52.jpg\" height=\"115px\" align=\"center\" border=\"0\" hspace=\"10\" \/><\/a><a href=\"https:\/\/media.pegasuslectures.com\/blog\/Picture53.jpg\" target=\"blank\" rel=\"noopener\"><img decoding=\"async\" src=\"https:\/\/media.pegasuslectures.com\/blog\/Picture53.jpg\" height=\"115px\" align=\"center\" border=\"0\" hspace=\"10\" \/><\/a><\/p>\n<p style=\"text-align: justify;\">We made arrangements to pay a local resident to cook for us each day.\u00a0 She is the wife of one of the clinic workers we are training.\u00a0 His name is Geordano.\u00a0 They have three adorable children \u2013 the littlest of whom stole my heart.\u00a0 She was afraid of me at first, but after seeing me in her house for a while, and a game of peekaboo, she started coming around.<\/p>\n<p style=\"text-align: center;\"><a href=\"https:\/\/media.pegasuslectures.com\/blog\/Picture54.jpg\" target=\"blank\" rel=\"noopener\"><img decoding=\"async\" src=\"https:\/\/media.pegasuslectures.com\/blog\/Picture54.jpg\" height=\"115px\" align=\"center\" border=\"0\" hspace=\"10\" \/><\/a><a href=\"https:\/\/media.pegasuslectures.com\/blog\/Picture55.jpg\" target=\"blank\" rel=\"noopener\"><img decoding=\"async\" src=\"https:\/\/media.pegasuslectures.com\/blog\/Picture55.jpg\" height=\"115px\" align=\"center\" border=\"0\" hspace=\"10\" \/><\/a><\/p>\n<p style=\"text-align: justify;\">Here is a picture of the family\u2019s pet turtle. \u00a0It had a string attached to it\u2026I guess in case the kids want to take it for a walk\u2026?\u00a0 Also notice a baby chick which seem to feel right at home wobbling into the house.\u00a0 I found the contrast of the baby chick with the Barbie-style doll amazing \u2013 two worlds colliding.\u00a0 As you can see, life is very different in the rural Amazon from what we are accustomed.<\/p>\n<p style=\"text-align: center;\"><a href=\"https:\/\/media.pegasuslectures.com\/blog\/Picture56.jpg\" target=\"blank\" rel=\"noopener\"><img decoding=\"async\" src=\"https:\/\/media.pegasuslectures.com\/blog\/Picture56.jpg\" height=\"115px\" align=\"center\" border=\"0\" hspace=\"10\" \/><\/a><a href=\"https:\/\/media.pegasuslectures.com\/blog\/Picture57.jpg\" target=\"blank\" rel=\"noopener\"><img decoding=\"async\" src=\"https:\/\/media.pegasuslectures.com\/blog\/Picture57.jpg\" height=\"115px\" align=\"center\" border=\"0\" hspace=\"10\" \/><\/a><\/p>\n<p style=\"text-align: justify;\">For breakfast we had a fried egg and some platinos (plantains).\u00a0 As I mentioned in my last blog, Jens is also a visitor here since he works at the <a href=\"https:\/\/www.wfpiweb.org\/Portals\/7\/Outreach\/CentrodeSaludSantaClotilde(CSSC).compressed%20(3).pdf\">hospital in Santa Clotilde<\/a> but is here training on <a href=\"http:\/\/mimasngo.org\/our-project\/\">Volume Scanning Imaging (VSI)<\/a>. \u00a0He seems to have a very fun-loving personality.\u00a0 Unfortunately, I do not understand as much of his Spanish as I would like.\u00a0 I am finding that there are some people that are easier for me to understand than others.\u00a0 This may be a function of individual dialects as well as unique pronunciations. As you can see in the picture, Jens is always smiling.<\/p>\n<p style=\"text-align: center;\"><a href=\"https:\/\/media.pegasuslectures.com\/blog\/Picture58.jpg\" target=\"blank\" rel=\"noopener\"><img decoding=\"async\" src=\"https:\/\/media.pegasuslectures.com\/blog\/Picture58.jpg\" height=\"115px\" align=\"center\" border=\"0\" hspace=\"10\" \/><\/a><a href=\"https:\/\/media.pegasuslectures.com\/blog\/Picture59.jpg\" target=\"blank\" rel=\"noopener\"><img decoding=\"async\" src=\"https:\/\/media.pegasuslectures.com\/blog\/Picture59.jpg\" height=\"115px\" align=\"center\" border=\"0\" hspace=\"10\" \/><\/a><\/p>\n<p style=\"text-align: justify;\"><a href=\"https:\/\/media.pegasuslectures.com\/blog\/Picture60.jpg\" target=\"blank\" rel=\"noopener\"><img decoding=\"async\" src=\"https:\/\/media.pegasuslectures.com\/blog\/Picture60.jpg\" height=\"115px\" align=\"right\" border=\"0\" hspace=\"10\" \/><\/a>For lunch we had some roasted chicken (cooked over an open fire) and some rice with a piece of <a href=\"https:\/\/www.webmd.com\/diet\/health-benefits-yuca\">Yuca<\/a> (pronounced YOO-ka).\u00a0 It tasted very much like a potato \u2013 but more flavorful.\u00a0 I am now seeing that the cooking hearth seems to be a mostly standard home accessory\u2026an open fire pit built into the kitchen floor.\u00a0 They have sand or dirt below the firepit as protection against igniting the wooden floors.\u00a0 It seems that there is really nothing that prevents the side boards from catching other than where they place the wood to be burned within the pit.\u00a0 They place iron grating above the fire when cooking or keeping foods warm.<\/p>\n<p style=\"text-align: justify;\">On our way back to the maternity house we met Dommi.\u00a0 After hugs and kisses\u2026a kiss on one cheek: the Peruvian\/Spanish way\u2026a kiss on both cheeks: the Italian way\u2026and three kisses (one side, the other side, and then back again): the Polish way. \u00a0Dommi commented that the Polish way \u201cis clearly best as you get three kisses.\u201d\u00a0 We thanked her profusely for the towels to which she quipped \u2013 \u201cI have the only five-star hotel around\u201d, laughing at her own self-deprecating humor.\u00a0 Dommi was there to announce the award of fifteen scholarships by <a href=\"https:\/\/sanjosedelamazonas.org\/santa-clotilde\/\">the Diocese<\/a> for local students to study <a href=\"https:\/\/en.wikipedia.org\/wiki\/Iquitos\">in Iquitos<\/a>.\u00a0 The plan is that the educated students would come back afterward and work in Angoteros.\u00a0 This education opportunity is very significant, as most families cannot afford to send their kids to schooling more advanced than our equivalent of middle school, and also because most families want to keep their kids with them to help work the land.\u00a0 I believe this scholarship plan is also a means of changing the culture so that people start to value education more by seeing how it can improve the lives of individuals and the community.<\/p>\n<p style=\"text-align: justify;\">We were amazed when we arrived in the house to see a large-screen television with 7 or 8 kids around it.\u00a0 Apparently, neighborhood children join their friends to watch television during the 4-hour period of grid electricity.\u00a0\u00a0 They were watching <a href=\"https:\/\/www.imdb.com\/title\/tt0092455\/\">Star Trek: The Next Generation<\/a>, which originally broadcast in the 1990s.\u00a0 The show was in English but there were Spanish subtitles.<\/p>\n<p style=\"text-align: justify;\">A little more about our training today.\u00a0 By late afternoon, we finally got down to the <a href=\"http:\/\/mimasngo.org\/our-project\/\">VSI training<\/a>.\u00a0 We were able to get a portable generator to use, with only a few issues, the first being that we only had about twenty minutes worth of gasoline.\u00a0 Eventually that issue was resolved \u2013 how exactly remains a mystery.\u00a0 The next challenge was how to connect electricity to the maternity house.\u00a0 The \u201cportable\u201d generator was very heavy and the staff decided to move it the shortest distance possible, which put it outside <a href=\"https:\/\/www.researchgate.net\/profile\/Leslie-Trujillo\">Leslie\u2019s<\/a> room.\u00a0 Geordano stripped the wires with a 2-foot-long machete.\u00a0 Yes, you read that correctly \u2013 the local clinic staff is very resourceful and find ways to make things work.\u00a0 Geordano twisted the wires and stuck the plus and minus leads into the prongs of a wall-socket.\u00a0 The lights came on but for some reason there was no electricity to the plugs in our training room.\u00a0 Eventually we found that one of the breakers was tripped and we were good to go.<\/p>\n<p style=\"text-align: justify;\"><a href=\"https:\/\/media.pegasuslectures.com\/blog\/Picture.jpg61\" target=\"blank\" rel=\"noopener\"><img decoding=\"async\" src=\"https:\/\/media.pegasuslectures.com\/blog\/Picture61.jpg\" height=\"115px\" align=\"left\" border=\"0\" hspace=\"10\" \/><\/a><a href=\"https:\/\/media.pegasuslectures.com\/blog\/Picture62.jpg\" target=\"blank\" rel=\"noopener\"><img decoding=\"async\" src=\"https:\/\/media.pegasuslectures.com\/blog\/Picture62.jpg\" height=\"115px\" align=\"right\" border=\"0\" hspace=\"10\" \/><\/a>We taped a bed-sheet to the wall to act as a screen.\u00a0 We brought in a small projector, and along with a few chairs and my bed to sit on, plus my laptop, we had an instant classroom.\u00a0 My talk was on basic <a href=\"https:\/\/test.pegasuslectures.com\/upi6e.php\">ultrasound physics<\/a>, followed by the theory of <a href=\"http:\/\/mimasngo.org\/our-project\/\">VSI<\/a>, and the differences between <a href=\"http:\/\/mimasngo.org\/our-project\/\">VSI<\/a> and traditional ultrasound.\u00a0 There were 8 people in attendance \u2013 including the three local clinicians who will be performing the protocols and the one person from <a href=\"https:\/\/www.wfpiweb.org\/Portals\/7\/Outreach\/CentrodeSaludSantaClotilde(CSSC).compressed%20(3).pdf\">Santa Clotilde<\/a> (Jens) who will do the same down there.\u00a0 The others included a young doctor, Franciet (spelling may be way wrong here) who is at the end of her program for which she has to perform one year of medical practice in a remote location.\u00a0 In two months\u2019 time, she will leave and a new doctor will start his or her rotation here.\u00a0 She really wanted to attend the lectures and training \u2013 and we of course are thrilled to include as many people as possible.\u00a0 We actually used the panoramic picture (included below) as a prop to explain how speed matters when performing a VSI sweep.<\/p>\n<p style=\"text-align: justify;\"><a href=\"https:\/\/media.pegasuslectures.com\/blog\/Picture63.jpg\" target=\"blank\" rel=\"noopener\"><img decoding=\"async\" class=\"aligncenter\" src=\"https:\/\/media.pegasuslectures.com\/blog\/Picture63.jpg\" height=\"115px\" align=\"center\" border=\"0\" hspace=\"10\" \/><br \/>\n<\/a><a href=\"https:\/\/media.pegasuslectures.com\/blog\/Picture64.jpg\" target=\"blank\" rel=\"noopener\"><img decoding=\"async\" class=\"aligncenter\" src=\"https:\/\/media.pegasuslectures.com\/blog\/Picture64.jpg\" height=\"115px\" align=\"center\" border=\"0\" hspace=\"10\" \/><\/a><br \/>\n<a href=\"http:\/\/mimasngo.org\/our-team\/\">Brian<\/a> gave a quick lecture on the <a href=\"http:\/\/mimasngo.org\/our-mission\/\">history of MIMAs<\/a> and showed some examples of <a href=\"http:\/\/mimasngo.org\/our-project\/\">VSI scans<\/a>.\u00a0 At that point we were ready to demo the first protocol which is for right upper quadrant.\u00a0 This protocol allows for diagnosing issues with the gall bladder, pancreas, liver, and bile ducts.\u00a0 It consists of six sweeps with the patient in various positions to acquire the optimal images.\u00a0 Tomorrow, the trainees will begin practicing the protocol themselves.\u00a0 After that, we will begin the OB protocol.<\/p>\n<p style=\"text-align: justify;\">We were pleased with today\u2019s outcome \u2013 but really wish we\u2019d had a fan to help keep the inside temperature down.\u00a0 We tried buying one in <a href=\"https:\/\/en.wikivoyage.org\/wiki\/Maz%C3%A1n\">Maz\u00e1n<\/a>, the town in which the boat stopped two days ago, but there was only one fan to be had \u2013 and it was so large that we did not think we would be able to carry it.\u00a0 We were rethinking that decision now.<\/p>\n<p style=\"text-align: justify;\"><a href=\"https:\/\/media.pegasuslectures.com\/blog\/Picture65.jpg\" target=\"blank\" rel=\"noopener\"><img decoding=\"async\" src=\"https:\/\/media.pegasuslectures.com\/blog\/Picture65.jpg\" height=\"115px\" align=\"right\" border=\"0\" hspace=\"10\" \/><\/a>Closing out, as power goes off in 31 minutes, one thing is for sure, <a href=\"https:\/\/www.citypopulation.de\/en\/peru\/loreto\/maynas\/1601100016__angoteros_monterrico\/\">Angoteros<\/a> locals are a lot tougher than we are.\u00a0 But I am thankful for the cool showers, for the treatment we have had, and the special accommodations that they have provided\u2026which goes way beyond what most people have here.<\/p>\n<p>I am including a picture of my room when
